Jerry Keith Mayes 63Mr. Jerry Keith Mayes, age 63, of North Columbia Avenue, Campbellsvile, died Wednesday, December 1, 2004, at his home after an illness.He was preceded in death by his father Tommy Mayes. He was a member of the First Methodist Church in Campbellsville.He is survived by his mother, DorothyRoberts Mayes. He also is survived by three uncles, George and Claude Mayes of Louisville, T. C. Roberts of Atlanta, Georgia, one aunt, Nellie Graham of Glasgow, along with several cousins and a host of friends.Funeral services for Mr. Jerry Keith Mayes will be conducted at 2:00 P.M., Friday, December 3, 2004 at the Lyon DeWitt Funeral Home with burial in the Brookside Cemetery.Rev.
